When calling for an appointment, please be prepared to provide the patient's name, address, phone number, birth date, type of insurance, reason for appointment, and referring physician, if applicable. Our phone lines are open from 9:00 a.m. to 5:00 p.m. Monday-Thursday, 9:00 a.m. to 12:00 p.m. on Fridays. All prescription refill requests should be done through the pharmacy by fax.
